Supervised Independent Living (SIL) Case Manager

JOB SUMMARY

The Supervised Independent Living (SIL) Case Manager provides support and direction for youth. Develops individualized life skills program for each youth. Demonstrates and teach skills what is necessary for youth to live independently. Locates appropriate community resources and establishes links. Advocates on behalf of youth. Implements service plans relative to emancipation. Coordinates ancillary services with the youth. Acts as a resource to Tabor staff regarding adolescent/independent living issues. Hours are from 9am – 5pm Monday through Friday, but may vary according to organizational needs. Regular travel required.

EDCATION

Bachelor’s degree in Social Work or related field.

EXPERIENCE

Two (2) years of experience working within child, youth and family service systems.

KNOWLEDGE, SKILLS & ABILITIES

· Strong written and verbal communication skills

· Skills with Microsoft Office applications including Word, Excel and PowerPoint

· Knowledge of PA Code 55 3680s, 3700s, and the Philadelphia DHS SBH Scope of service

· Knowledge and skills in cultural diversity

· Ability to achieve desired performance outcomes

· Ability to assess and problem solve

· Capable of working responsibly with highly confidential information

· Ability to multi-task and demonstrate good organizational skills

· Ability to work independently as well as part of a team

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Maintains caseload of independent living youth.

· Contacts with each youth as mandated by SIL regulations.

· Develops individual service plan for all youth in compliance with regulations.

· Develops all resources necessary to service provision including location of apartment.

· Works with biological families to facilitate goals of individual service plan.

· Coordinates all systems/services involved with youth.

· Collaborates with Tabor staff, referring agency and all other professionals.

· Develops of life skills plan for each youth.

· Develops group structure to present life skills/support to all older adolescents.

· Maintains complete client file, including ongoing written record of all client/collateral contracts; mandated reports and plans; and any other written work necessary for regulation compliance.

· Develops and implement discharge plan for youth.

· Assess youth and their relationships with biological family and peers, taking into consideration the educational, emotional, psychological and physical needs of client.

· Provides counseling, support and advocacy for youth.

· Provides crisis intervention and resolution as needed with individual caseload and as part of the SIL team as necessary.

· Assesses biological family members and their relationship to youth and their ability to provide support during and after independent living.
