Director of Family, Health, & Wellness (Full-Time)

Tabor Community Partners is currently seeking a Director of Family, Health, & Wellness that desires to make an impactful change amongst the under served community located in the Germantown section of Philadelphia. This position will give you an opportunity to provide strategic leadership, directing and monitoring the Prevention Services teams, and also planning and designing a variety of projects that are geared to support our youth, families, and our community.

The Director of Family, Health, and Wellness position is also requiring a clinical approach to some of the job responsibilities, by providing care assessments in coordination with case management services; conducting wellness visits, collaborating with physicians, pharmacists and other clinical providers to coordinate care and services for families. Public speaking and clinical training and education for the Prevention Services team are also required.

To qualify for this position you must have a degree in Nursing from an accredited college and/or a Master’s Degree in Public Health, Healthcare Policy and Administration, or Social Work with two (2) years experience in a personal, home health, family care, or behavioral health environment. 2 years’ experience working in a health care setting as a Healthcare Coordinator or Patient Navigator will also be considered.

This is a full time position Monday-Friday 9am-5pm. Hours may vary based on job functions and responsibilities.
