CUA Care Coordinator

The CUA Care Coordinator works in collaboration with the CUA Case Management teams in its continuous partnership with children and youth with special health care needs, and medical complexities while working alongside their family/caregiver(s).

 

The CUA Care Coordinator will provide support, guidance, and assistance to the clients’ and families with community resources. With the goal of enhancing their health and well-being.

 

This position requires a bachelor's degree in healthcare or in a field of specialization (public health) with 3-5 years' experience in clinical or community resource settings.

 

Care coordination and/or case management experience is desirable.

 

Job Type: Full-time

Pay: $40,000.00 per year
