NOTICE: VENUE SELECTION

By accepting and maintaining placement on behalf of an individual(s) at Tabor Children’s Services, Inc. (the “Placement”), you: 

(1) consent and agree, effective as of the date of admission to the Placement, to the exclusive jurisdiction of the Court of Common Pleas of Bucks County, Pennsylvania, to hear and determine all disputes and matters arising out of, relating to, or pertaining to the Placement, any and all agreements relating to the Placement, and any and all allegations of wrongdoing, negligence or willful misconduct by Tabor Children’s Services, Inc., its employees, trustees, officers, agents, related parties or affiliates, relating to, pertaining to or concerning the individual who is the subject of the Placement (“Related Matters”); and 

(2) consent and agree, effective as of the date of admission to the Placement, that the laws of the Commonwealth of Pennsylvania shall govern the enforcement and interpretation of the Placement and Related Matters without regard to conflict of laws principles. 

It is further understood and agreed that the above provisions are key components of the consideration for the agreement of Tabor Children’s Services, Inc. to the Placement and shall remain in full force and effect after the Placement is ended and unless and until you and Tabor Children’s Services, Inc. agree in writing to its modification or termination.
