Summary of Health Plan Benefits

We are pleased to provide you with a summary of our benefits that describe Tabor’s comprehensive benefits offered to you and your eligible family members.  This plan year runs from November 1, 2023 – October 31, 2024. The company sponsored benefits offered during a plan year include medical/prescription drug, dental, vision, and voluntary health plan benefits. Tabor also offers Life Insurance, Accidental Death & Dismemberment (AD&D), and Long-Term Disability coverage at no cost to you.

Tabor also offers you and your eligible family members voluntary benefits including Life Insurance, AD&D, Short-Term Disability, Accident Insurance and Critical Illness insurance. These plans are offered to the employee at minimal cost and deducted from your bi-weekly paycheck

BENEFIT ENROLLEMENT

New hires are eligible to enroll in benefits on the 1st day of the month following 60 days of continuous employment.  New hires will receive an email notification from Paycom 30 days prior to their benefit effective date prompting you to log into the Paycom Employee Self-Service to begin the enrollment process.

Homestead/Claim Watcher (Smart Health Plans) 

Plan Description – is a MultiPlan PHCS—Practioner Only national network. The PHCS network offers access in all states to over 700,000 healthcare professionals, including both primary and specialist practitioners. Tell your provider that your coverage is through the Employer’s Open Access Self-Funded plan.  If they have additional questions, your provider can call customer service at 888-446-3327. 

 

US-Rx Care Prescription Drug Plan (included with Homestead/Claim Watcher medical plan) 

If you are enrolled in one of the Tabor Services, Inc. medical plans, you are automatically enrolled in the corresponding prescription drug plan, US-Rx Care.  Visit the benefits portal listed below for details on the prescription drug plan and how you can search for lowest-cost pharmacies on your US-Rx Care member portal.  

 

Delta Dental PPO

If you are enrolled in one of the Tabor Services, Inc. medical plans, you are automatically enrolled in the corresponding prescription drug plan, US-Rx Care.  Visit the benefits portal listed below for details on the prescription drug plan and how you can search for lowest-cost pharmacies on your US-Rx Care member portal.  

 

Delta Dental PPO 

This preferred provider plan offers the convenience and flexibility of visiting any licensed dentist, anywhere. Covered services are paid based on a percentage.  If, for example, fillings are covered at 80%, you pay the remaining 20%. 

 

Delta Dental DHMO 

With this plan, you and each of your covered dependents are required to choose a participating Primary Care Dentist (PCD) to coordinate your care.  If you require specialty care, your PCD can refer you to a network specialist for covered services, however, you may visit a participating orthodontist without a referral.  See the 2023-2024 Employee Benefits Guide for additional information of plan coverage. 

 

National Vision Administrators (NVA) - Vision  

NVA is all about easy! If you elect to enroll in this vision plan, you will receive a Welcome Packet which will contain two ID cards, a listing of 10-12 provides near your home zip code, a benefit summary, information on contact lens mail order and information on LASIK surgery. 

 

Symetra Voluntary Group Life/AD&D Insurance and Long-Term Disability  

This voluntary benefit covers you for life insurance/AD&D at 2 times your Basic Annual Earnings, up to a maximum of $650,000.  The premium for this benefit is completely paid for by Tabor Services, Inc. at no cost to you!  

Symetra – Voluntary benefits are 100% employee paid!! 

Voluntary Life Insurance/AD&D, Employee, Spouse & Child(ren) 

You can choose from $5,000 to $500,000 in increments of $5,000, not to exceed 5 times your Basic Annual Earnings, of additional life insurance for you and your loved ones. No medical questions asked up to the guaranteed issue amount of $150,000.  For your child (ren) the basic amount of life insurance is limited to $10,000.  Based on your elections, premiums are post-tax  and deducted from your bi-weekly paycheck.   

Voluntary Short-Term Disability Insurance are 100% employee paid!! 

If you depend on your income to pay the bills, you need to seriously consider buying disability income insurance.  It can help you maintain your current lifestyle and help protect you and your family from going into serious debt. 

Additional Voluntary Benefits at a Glance:

  • SL Accident – offers a $50 annual wellness reimbursement

  • SL Critical Illness – offers a $50 annual wellness reimbursement 

Based on your elections, premiums are post-tax and deducted from your bi-weekly paycheck.
